The information collected will be used to create a profile of the potential Board membership in order to identify gaps and to achieve a Board that has the attributes and perspectives that are required. Completion of the survey is based on how you self-identify. Information specific to individuals will not be disclosed other than to the Nominations/Interviewing panel.

This matrix has been developed to assist the nominating committee to recruit new board members and to identify the skills and strengths of existing board members. Please indicate your areas of knowledge, skills and experience by checking off the relevant square boxes in the form below. It is not expected that you possess knowledge or skills in all areas set out below.

  • NONE – no understanding, experience, knowledge or skill
  • BASIC – general or fundamental understanding, experience, knowledge or skill

As a CHC, we focus on working with individuals who may face barriers to accessing services, including individuals who are isolated, those who experience stigma and discrimination, and individuals with health and other challenges not addressed in other environments. Our vision is for a community where we work together to achieve equity, belonging, health and wellbeing for all.
